Name Change or Passport Correction

If you have changed your name or if you have discovered a mistake in your passport, you may apply to have this change reflected in your passport. Please note that U.S. passports cannot be amended to change items appearing on the data page; you will therefore need to apply for a new passport.

If your name is changed or the mistake is discovered within one year after your passport was issued, please submit:

  • Current passport (issued less than one year ago)
  • Completed form DS-5504
  • One photograph 5 cm x 5 cm (not older than six months)
  • Documentation of name change (e.g. original or certified copy of marriage certificate, divorce decree or court order, including translation if original is not in English) or evidence of correct biographic information (original birth certificate, including translation if original is not in English, or previous U.S. passport)

Fees: no fee is charged for this service.

If the passport is issued more than a year ago, please follow procedure for passport applications. Please submit documentary evidence of name change (e.g. original or copy of marriage certificate, divorce decree or court order) or evidence of correct biographic information (birth certificate or previous U.S. passport). You will need to pay the appropriate fee for this service.
